New Hire Employee Information Form

New hire employee form is the key documents of the personnel file. It is Human Resource's responsibility to collect the below-mentioned information with valid documents to keep it in safe custody for future reference.

Indeed the most important information of the respective employees is personal, educational, past experience, training, and dependants name including their date of birth, gender and other related information for inclusion in the organization’s health/medical insurance policy if employee married.

A new employee should provide a minimum of three references from the well-known person are mandatory to provide an employee after selection.

Please note that employee information is an important service record of each employee, an employee should fill this form completely and accurately. The employer shall not entertain such information, which cannot be substantiated by valid supporting documents/certificates. The attestation of documents should be from a competent authority and notables of the respective vicinity.
